This group of forms are used in transferring items involved in an estate. This would include forms for completing estate work along with information sheets to be filled in and mailed, or sent by facsimile transmission (732-506-5087), to the surrogate's court in order to expedite the preparation of estate papers.
- 1a. Application for Voluntary Discharge of Executor or Administrator C.T.A.pdf
This form is used when a Will has been previously probated in the Ocean County Surrogate’s Court, the appointed personal representative (Executor or Administrator C.T.A.) no longer wishes to serve in that role, and another person wishes to assume the responsibilities of administering the Estate. This application must be filed along with the Consent of all interested parties to the estate, and an Application for Appointment of Successor Executor or Substitute Administrator C.T.A.

This form is used when a Will has been previously probated in the Ocean County Surrogate’s Court, the appointed personal representative (Executor or Administrator C.T.A.) no longer wishes to serve in that role, and another person wishes to assume the responsibilities of administering the Estate. This application must be filed along with the Consent of all interested parties to the estate, and an Application for Voluntary Discharge of Executor or Administrator C.T.A.

This form is used when and administrator has been previously appointed in the Ocean County Surrogate’s Court, he or she no longer wishes to serve in that role, and another person wishes to assume the responsibilities of administering the Estate. This application must be filed together with the Consent of all interested parties to the estate, and an Application for Appointment of Substitute Administrator.

This form is used when and administrator has been previously appointed in the Ocean County Surrogate’s Court, he or she no longer wishes to serve in that role, and another person wishes to assume the responsibilities of administering the Estate. This application must be filed along with the Consent of all interested parties to the estate, and an Application for Voluntary Discharge of Administrator.
- 3. Affidavit of Residence
This form is used when the death certificate shows the decedent's address is incorrect. The person who can sign this form must be an uninterested party to the estate. (Such as a good friend or neighbor who does not inherit from the estate)

- 10. Notice of Probate/Proof of Mailing
As Executor/Executrix and/or Administrator/Administratrix of the Decedent you are responsible to notify in writing all Beneficiaries and all Next of Kin that a Will has been probated. (This does include all immediate Next of Kin even if they are not named as Beneficiaries in the Will.)
- 12.Refunding Bond and Release Form with Sample
This form is used to show that you have received your part of any estate and wish to release the Executor or Administrator from bond, if one is necessary. This form must be signed in the presence of a notary public. The sample form shows how the refunding bond and release form should be filled out.
